Employment Application Packet

 

Please complete application and turn in with all requested documentation, incomplete applications will not be considered.

 

Attach your Resume with the Employment Application to make your Packet complete.  Include three (3) personal references in your resume.

The following documents are required to submit with your employment Application:

 

DRIVING RECORD

Can be obtained from the New Mexico Department of Motor Vehicles at no cost.

 

CRIMINAL BACKGROUND HISTORY

Can be obtained from the McKinley County Sheriff's Office.

 

CURRENT TUBERCULOSIS TEST

Can be obtained from:

-Northside Clinic @ a sliding fee

-Gallup Indian Medical Center (for Native Americans)

-RMCH College Clinic.  Make an appointment, you will need to see a physician.  Fee is $16.00

 

DRUG TEST

Can be obtained from:

-Gallup Indian Medical Center (for Native Americans)

-RMCH College Clinic.  ( At own expense)

 

FINGERPRINTS

Are required by law, they can be obtained at the local sheriff's office at your own expense, upon hire date.

 

IF YOU DO NOT ATTACH ANY OF THE ABOVE DOCUMENTS WITH YOUR APPLICATION, IT WILL BE CONSIDERED INCOMPLETE AND WILL NOT BE CONSIDERED.
